SAFE ENVIRONMENT FORMS - ADULTS
- Safe Environment Forms – Completed by all parish volunteers. The purpose of this documentation is to keep a record of volunteers for audit purposes and for Archdiocese record keeping purposes. A volunteer application is good for 5 years.

- Volunteer Background Check Instructions & Forms Anyone that deals directly with the youth and/or the vulnerable must have a completed background check on file. The background check is good for 5 years.

MINOR FORMS (Between 13-17 years of age)
- Volunteer Application for Minors – Completed by all minor parish volunteers. The purpose of this documentation is to keep a record of volunteers for audit purposes and for Archdiocese record keeping purposes. A minor volunteer application is good for 5 years.
- School Reference for Minor Volunteers – In addition to the above application for minors, a school reference must be submitted for each teen volunteer.
(Under the age of 13 requires parent written permission)
