This program serves our middle and high school youth. Programs include faith formation, spirituality, community service, and just plain fun! Each Year, teens must fill out an annual medical release form, This will be good for one year. If any information changes, updates should be made.** Attached is the form, and hard copies will be available in the Narthex** Please return all forms to the Faith Formation Department attention: Jane Hernandez.
